Anthem Blue Cross has launched a new population health system connecting patient information among Vivity hospital systems.
The new system will give providers access to robust information about patients, including claims history and clinical documents such as discharge summaries across the continuum of care, the company said.
Vivity, a product partnership between health plan and seven hospital systems, provides a virtually integrated healthcare system experience that delivers high-quality, cost-effective care and coverage.
The new population health management platform allows for greater transparency into a patient's medical history and promotes more effective coordination of care across all Vivity partners. It also provides analytics, making it easier for providers to share best practices and continuously improve quality across the system.
