Caradigm, a healthcare analytics and population health company, has signed an agreement with MEDai, a company that offers predictive clinical analytic solutions.
The agreement is designed to give healthcare organizations the comprehensive set of tools they need to effectively manage risk, optimize care management, and improve the health outcomes of populations.
Under the agreement, Caradigm will enrich its population health solutions with the predictive clinical analytics capabilities of MEDai's solution, enabling customers to stratify patient populations according to severity, disease state, cost or risk profile.
This will allow risk-bearing organizations to use their network of resources to focus care on those who will benefit most, helping to create a positive impact on overall health outcomes and total cost of care.
When coupled with Caradigm Care Management software, providers can develop personalized care plans that specify problems, goals and interventions, and manage patients and conditions through timely interventions enabled by near real-time, event-based alerts.

MEDai's solution uses statistical algorithms to identify and risk-stratify patients based on a wide range of factors, including: severity of illness, patient motivation and compliance with evidence-based care guidelines, risk of inpatient admission or emergency department visit, and the total cost of care based on the number of conditions, prescriptions and type of care required.
