Creo Wellness, a provider of population health management (PHM) solutions, has signed a group purchasing agreement with HealthTrust and its CoreTrust division to offer employee wellness programs to member organizations.
Creo’s PHM solution is a comprehensive approach for employee wellness programs, healthcare systems’ population health outreach efforts, and other members.
Program participants have access to personalized, participatory and preventive experiences that can help them manage chronic diseases and risk factors, resulting in improved health for the individual, improved outcomes and lower healthcare costs for the plan.
“This agreement will allow member organizations to participate in a program that aims to improve employee health and help reduce healthcare costs,” said Peter Bernard, CEO of Creo.
Creo uses mobile-first technology with a backend that uses machine learning to personalize the experience. This approach aims to solve many of the problems that exist with older programs.
