Evolent Health partners with Community Care Cooperative in Massachusetts

Evolent Health and Community Care Cooperative (C3) have partnered to promote value-based care in Massachusetts for patients enrolled in the state's MassHealth Medicaid program and assigned to the C3 Accountable Care Organization (ACO).

C3 is a health care corporation formed to take responsibility for managing the cost and quality of health care for its members in the region.

"We are thrilled to partner with C3 in this innovative partnership that is closely connected to our mission as it will change the way care is delivered in Massachusetts," said Evolent CEO Frank Williams.

"By leveraging the best practices, knowledge and expertise of both sides, we believe we can impact the health and well-being of underserved populations by applying data-driven, population health approaches in the trusted, high-value settings of C3's FQHCs. We are excited to be working with C3 as our first FQHC-centric partner," Williams added.

The partnership combines Evolent's predictive analytics, technology infrastructure and clinical models with C3's integrated physical and behavioral health expertise in diverse local communities. Together, the organizations aim to build a value-based care delivery system that could serve a significant number of MassHealth beneficiaries.
