Evolent Health and Houston Methodist have partnered to drive value-based care in Texas for improving the health of Medicare patients.

Together, the organizations are taking a physician-led approach to deploying a value-based care technology infrastructure and clinical programs, including advanced analytics and predictive modeling, to help identify patients that might benefit from targeted clinical interventions early and with improved accuracy.
Through this closely-aligned collaboration, Evolent supports Houston Methodist in the development of Houston Methodist Coordinated Care (HMCC), a physician-led ACO.
