Summit Health, a health system serving the population of south central Pennsylvania, has implemented Wellcentive's suite of solutions and services for population health management.
Summit Health contracted with Wellcentive to implement a technology infrastructure that would enable its leaders and care providers to manage a wide variety of quality initiatives and risk-based contracts.

Summit Health implemented Wellcentive as part of its long-term strategic plan for managing risk across the care continuum. With the platform for population health management now live, the health system is prepared to participate in a variety of payer-sponsored quality initiatives.
Wellcentive will enable Summit Health to manage multiple pay-for-performance and risk-based contracts, such as the Highmark Quality Blue Physician Program. The health system is also preparing to participate in patient-centered medical home (PCMH) initiatives, including gaining certification through the Accreditation Association for Ambulatory Healthcare.
